Most respectfully I beg to submit that I have lost my original copy of the mark sheet for the high school Examination 2020. My Roll No. 5236 and I passed the examination in the first division as a regular candidate from Rajnandgaon.
Unfortunately, I lost my mark sheet during the transfer of my father. so I requested you kindly issue a duplicate copy of my mark sheet.
I have attached the following documents required for the issue of duplicate mark-sheet
- A Bank Draft of Rs. 100/- for fee
- An Affidavit for the loss of Mark-sheet.
Kindly issue the duplicate copy of my mark sheet at an early date at the following address.
